Appointments and Office Hours


 

Appointments can be made

• Monday 9-5:00
• Tuesday 8-3:00
• Wednesday 10-6:00
• Friday 8-3:00
  

 

To make the most of your time with us, the following steps should be taken:

• Please arrive 15 minutes early for your first appointment, so that you will have sufficient time to check-in, complete your new patient paperwork, remove make-up, and relax prior to your visit.
  
• Being on time for your appointment is greatly appreciated so that you are able to maximize the time you spend at your appointment, and so that our other patients are not inconvenienced. In fairness to our other patients, if you arrive late -- we regret that we may not be able to accommodate you on that day.
  
• In advance of your visit, you can print out the new patient paperwork online, and complete it prior to your visit.
  
• If your insurance plan requires a referral from your primary care physician, then you must obtain this referral before your appointment.
  Please bring your insurance card to all appointments.

Appointment Cancellation Policy

Your appointment is reserved especially for you. Should you need to cancel or change the date of your appointment, we require at least 24-hours notice as a courtesy to other patients seeking appointments. If less than 24-hours notice is given, if you miss an appointment, or if you are late - then a late cancellation fee or missed appointment fee will apply.
In fairness to other patients, if you arrive late -- we regret that we may not be able to accommodate you on that day -- and a late cancellation fee will apply.
